The pain is dull, or sore and uncomfortable, with stiffness, heaviness, and fatigue in the lower back and waist. The local skin is numb, rough or stiff, with dull sensation and widespread tenderness. Some patients can feel obvious tender points by themselves, while others cannot clearly point out the location of the pain. The pain has a traction reflex conduction phenomenon. Some back pain is reflected to the front chest and thorax, and some is reflected to the buttocks and legs. The lumbar and back muscles are in long-term tension and spasm. The affected area is hard when pressed and has various painful nodules in various forms, such as granular, cord-like or block-like. Treatment principle: soothe muscles and activate blood circulation, warm the meridians and dredge the collaterals, relieve contractures, resolve blood stasis and relieve pain.

Acupoints to be selected: Shenshu, Ganshu, Spleenshu, Yaoyangguan, Zusanli, Diaphragmshu, Jinsuo, Jiaji, Jianjing, Baliao and local tender points of lesions (Ashi points).

Techniques: kneading, pressing, plucking, holding, rubbing, tapping, etc.

Order of operations:

1. The patient lies prone, and the operator uses the palm or thenar and hypothenar eminences to gently knead, rub, push, and massage the lesion area and its surroundings for about five minutes to accelerate local blood circulation and dredge the meridians.

2. Use your folded palms or the base of your palms to massage the affected area and the surrounding area with slightly greater force for about three minutes to fully relax the local muscles.

3. Press the local acupuncture points (Ashi points) for about two minutes on each point to unblock Qi and blood and relieve pain.

4. Use your thumb or back joint to repeatedly tap, press, push and stroke the affected area with strong force for about five minutes. For positive reactions such as bulging, stiffness, thickening, nodules and lumps in the tendons, the treatment can be focused on. Following the circulation direction of the tendons, move them left and right, massage them back and forth, and push and stroke them up and down to straighten out the tendons and eliminate stagnation.

5. Use the thumb, index and middle fingers of both hands to lift and pluck the tendons three to five times. This can help separate adhesions and relieve spasms.

6. Rub, knead, and rub the affected area with your palms, and tap back and forth with your empty fists for about five minutes to further dredge the meridians and blood, and relieve symptoms such as numbness and contracture.
